CO-MANAGEMENT AGREEMENT
Co-management agreements with behavioral health providers and Urgent care facilities improve patient care. Our practice partners with other providers to make sure everyone on your care team knows what is going on with your care plan and has up-to-date information from your medical record. The purpose of the co-management agreement is to specify what each provider is responsible for in regards to your care. We uses care coordination agreements, when working with referring clinicians which define guidelines about the exchange of information, expedite timeliness and appropriateness of referrals and improves coordination of patient care. We employ this best practice model when sending and receiving referrals. Here are some practices we work with:

Behavioral/Mental Health
Lizzette Potthoff, LCSW, LCAS
is our go to provider for Mental health referrals . We have a co-management agreement in place with her and work closely when managing patients together. Important information about Lizzette includes the following :

Urgent Care
Sometimes our patients need help on the weekends or evenings when the office is closed. We recommend the following Urgent care facilities. They take appts and walk ins and are also helpful if you have a non-gyn problem like sinusitis or bronchitis. We have a co-management agreement with Med First Immediate Care & Family Practice.

Dr Motyka offers basic wellness, screening and primary gynecological care. If you have other medical problems and do not have a primary care provider, please see our page with links for PCPs we partner with frequently. Please let us know your primary care provider's contact information and what clinical information you would like us to share with them. Also, let us know if we need to consult your primary care provider prior to treating you. Let Dr Motyka know at the end of your visit if you would like a summary of care sent to your PCP.
